Placing an order

When placing an order for the services of the dental laboratory, you first need to fill in an order form. You can download the order form here. Please fill in the order form thoroughly and legibly and send it to the dental laboratory together with your dental impression.

In order for the dental laboratory to process the order, the order form must contain the following information:

• The name of the clinic, doctor and patient
• The content of the work according to the type of work (type of work, material, tooth shade, implant system, stage of work, and additional information if necessary)
• The desired dates for the completion of the work and/or the stages of work to be carried out

If we do not have enough information to carry out the work, it will remain on hold in the dental laboratory until we have received all the necessary information to proceed with the work. The deadline for completing the work will also be postponed by the waiting time.

The above information is also required when sending a digital impression.
